Hearthstone Hospice was founded by a team of healthcare professionals who believed end-of-life care could be more personal, more dignified, and more loving. That belief started with Hearthstone Communities, our home care network in the Southeast Michigan area, then grew into a hospice program built the same way — around people, not paperwork.
We care for the whole person — medically, emotionally, and spiritually — and we extend that same care to the families and caregivers walking this journey alongside them. Every patient is treated like a member of our own family, because to us, that’s exactly what they are.
How We Care For You
Compassionate Hospice Care Services
Hospice care is usually provided where the patient lives, although Hearthstone Hospice also offers inpatient care at a hospital or other facility in Macomb, Wayne, and Oakland Counties of the Greater Detroit Region.
We offer a full range of care levels — from routine home visits to inpatient and continuous crisis care — along with hands-on support, end-of-life planning, and grief counseling for the whole family.
Nursing Care
Our specialized nursing team delivers hands-on, custom care tailored to each patient’s condition and comfort needs.
Doctor Services
A dedicated Medical Director oversees every patient’s plan of care and remains available around the clock to guide treatment decisions.
Medication Management
We coordinate medications related to a patient’s terminal diagnosis, making sure prescriptions are managed and delivered without added stress on the family.
Spiritual Care
Our spiritual counselors meet patients wherever they are in their beliefs, offering comfort without judgment or a one-size-fits-all approach.
Medical Equipment & Supplies
From wheelchairs and hospital beds to catheters and bandages, we provide the equipment and supplies patients need, right where they live.
Medical Social Worker
Our social workers help patients and families build a day-to-day care plan while offering support through emotional, social, and financial stress.
